REALIDADE\u201d<\/h1>\n<p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; O mundo enfrenta a pandemia da COVID-19, causada pelo SARS-CoV-2, o qual teve seu surgimento em Wuhan no m\u00eas de dezembro de 2019, na China. O coronav\u00edrus \u00e9 capaz de causar uma S\u00edndrome Respirat\u00f3ria Aguda Grave (SARS), levando a \u00f3bito milhares de pessoas. Os sinais e sintomas mais comuns incluem cefaleia, febre, tosse, dispneia, diarreia e pneumonia. Contudo, com o aumento crescente de indiv\u00edduos infectados, novos sinais e sintomas est\u00e3o sendo descobertos, existindo uma varia\u00e7\u00e3o das manifesta\u00e7\u00f5es entre os indiv\u00edduos infectados.<\/p>\n<p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Em maio de 2020, estudos descreveram les\u00f5es em cavidade oral, como poss\u00edveis manifesta\u00e7\u00f5es da COVID-19. Foram observadas les\u00f5es ulceradas em l\u00e1bios; les\u00f5es compat\u00edveis com eritema multiforme; gengivite descamativa e m\u00faltiplas \u00falceras amareladas com um halo eritematoso (avermelhado) em regi\u00e3o de palato duro (c\u00e9u da boca), que se assemelhavam a les\u00f5es herp\u00e9ticas. Essas manifesta\u00e7\u00f5es podem estar associadas ao aumento de citocinas que podem resultar em danos teciduais, principalmente em mucosa. Apesar dessa explica\u00e7\u00e3o, n\u00e3o foi poss\u00edvel confirmar sua rela\u00e7\u00e3o com o novo SARS-CoV-2 (Carreras \u2010 Presas et al. 2020).<\/p>\n<p><img loading=\"lazy\" class=\"alignnone wp-image-922\" src=\"https:\/\/radiologiajardim.com.br\/wp-content\/uploads\/2020\/09\/1-1.png\" alt=\"\" width=\"414\" height=\"214\">&nbsp; <img loading=\"lazy\" class=\"alignnone wp-image-924\" src=\"https:\/\/radiologiajardim.com.br\/wp-content\/uploads\/2020\/09\/6.png\" alt=\"\" width=\"218\" height=\"237\"><img loading=\"lazy\" class=\"alignnone wp-image-923\" src=\"https:\/\/radiologiajardim.com.br\/wp-content\/uploads\/2020\/09\/3.jpg\" alt=\"\" width=\"214\" height=\"153\"><\/p>\n<p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; A disgeusia\/ageusia, s\u00e3o outros sintomas podem ser citados na cavidade oral, como que se caracteriza pela altera\u00e7\u00e3o ou aus\u00eancia do paladar, respectivamente, e a xerostomia, que compreende a sensa\u00e7\u00e3o de boca seca, muitas vezes em decorr\u00eancia da diminui\u00e7\u00e3o do fluxo salivar.<\/p>\n<p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Uma das prov\u00e1veis explica\u00e7\u00f5es dessa sintomatologia se deve ao fato de as papilas gustativas possu\u00edrem receptores gustativos que s\u00e3o amplamente distribu\u00eddos na cavidade oral, principalmente no dorso de l\u00edngua. Mas al\u00e9m dessas papilas, o dorso lingual tamb\u00e9m \u00e9 repleto de receptores da Enzima Conversora da Angiotensina 2 (ECA2), a qual o SARS-CoV-2 se liga para entrar na c\u00e9lula hospedeira. Dessa forma, os receptores gustativos tamb\u00e9m podem ser infectados, e consequentemente haver\u00e1 a perda do paladar. A disgeusia\/ageusia j\u00e1 s\u00e3o consideradas sintomas caracter\u00edsticos da COVID-19, mas ainda n\u00e3o h\u00e1 tratamento espec\u00edfico para esses sintomas. Contudo, s\u00e3o revers\u00edveis e desaparecem duas semanas ap\u00f3s a recupera\u00e7\u00e3o do paciente.&nbsp;<\/p>\n<p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Sobre a xerostomia, acreditam estar interligada com a pr\u00f3pria altera\u00e7\u00e3o no paladar, pois o sabor \u00e9 um dos principais estimuladores da produ\u00e7\u00e3o salivar. Em um estudo, foi apontado que 50% dos indiv\u00edduos infectados que apresentavam altera\u00e7\u00e3o de sabor tamb\u00e9m demostraram ter hipossaliva\u00e7\u00e3o e xerostomia.<\/p>\n<p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Por se tratar de uma novo v\u00edrus, a ci\u00eancia ainda est\u00e1 no processo de compreender como o mesmo funciona e se manifesta no corpo humano, pois, o consenso na comunidade cient\u00edfica que a cavidade oral pode apresentar ind\u00edcios de diversas doen\u00e7as sist\u00eamicas, bem como rea\u00e7\u00f5es a agentes externos.&nbsp;At\u00e9 o momento, o n\u00edvel de evid\u00eancia cient\u00edfica \u00e9 insuficiente para apoiar a caracteriza\u00e7\u00e3o de uma manifesta\u00e7\u00e3o oral espec\u00edfica, ou mesmo n\u00e3o espec\u00edfica, do COVID-19.&nbsp;Assim, neste momento \u00e9 dif\u00edcil afirmar que a ocorr\u00eancia de les\u00f5es orais est\u00e1 diretamente relacionada \u00e0 COVID \u2010 19.&nbsp;Mais estudos e investiga\u00e7\u00f5es precisas s\u00e3o necess\u00e1rios para confirmar se as les\u00f5es est\u00e3o associadas ao v\u00edrus SARS-CoV-2 ou outras infec\u00e7\u00f5es virais orais, como HSV-1, com rea\u00e7\u00f5es de hipersensibilidade induzida por drogas ou com qualquer outra condi\u00e7\u00e3o de defici\u00eancia imunol\u00f3gica.
